Complete pancreatic duct disruption in an isolated pancreatic injury: successful endoscopic management
نویسندگان
چکیده
Isolated pancreatic injury is uncommon and the management is challenging. Presentation is mostly delayed in such cases. We report a case of 26-year-old male, who was referred to our centre due to blunt abdominal trauma of 48 h duration with pain abdomen and vomiting. He was haemodynamically stable and was subjected to endoscopy after few hours of admission, but failed to cannulate the main pancreatic duct. Successful stenting was performed after 3 days of admission in the second attempt. He made an uneventful recovery and remains well 2 months after the injury. Management of isolated pancreatic injuries with complete duct disruption is challenging. Endoscopic management is an attractive minimally invasive option which avoids the need for surgery. Further studies are required regarding the selection of patients, safety and long-term outcome.

CONTEXT A pancreatic fistula is the most common complication of pancreatic injury. Although spontaneous closure of pancreatic ductal disruption has been reported, surgical treatment is accepted as the single most carried-out intervention in major ductal injury. We report a case of pancreatic duct disruption due to a bullet injury managed successfully by endoscopic pancreatic duct stenting. CA...

Recently, the diagnostic evaluation of pancreatic injury has improved dramatically. On the other hand, it is occasionally difficult to diagnose pancreatic injury, because there are no specific signs, symptoms, or laboratory findings. Radiological imaging also often fails to identify pancreatic injury in the acute phase. Delayed diagnosis results in significant morbidity and mortality. Most case...

An 11-year-old boy was admitted to the emergency department with severe abdominal pain and vomiting after falling off his bicycle. A round hematoma was noted in the left hypochondrium consistent with bicycle handlebar trauma. Blood analysis showed mild leucocytosis and marked elevation of lipase (2507U/L) and amylase (1331U/L). Abdominal ultrasound excluded spleen injury, and thoracic X-ray was...
